Copyright release form

What do you guys use if anything when someone wants a corporate logo digitized for garments. Is there a special form you give them to fill out or do you have them type a letter on company letterhead stating it is ok to reproduce the logo for garments etc...

I would also love a sample form, if it's out there! I just had an inquiry regarding this yesterday from an employee that wants to get the company logo on his own shirt, at his own cost.

I don't have a form but my order form and website says if your company is a franchise, they must provide a letter or email from the company headquarters giving permission to use the logo on apparel.
I do ask for the letter or email before proceding with the order. No letter from the company, no order is written up.
I would rather loose the sale than get sued by some company that has tons of more money than me.

I have a couple of samples that I got so far. If you send me your email I will send them to you.
I got one off the Impressions website that I used right away but a fellow NNEP member sent me theres to look at as well. I will contact them to see if it is ok to send you theirs if you want that one as well.

Can you share the form with me also? I just received a request from a customer to embroider his vest & chap with Harley Davidson, and I do not have license. Any suggestions?

A release form means nothing unless it is from the trademark/copyright holder. There is a 100% chance HD will not sign it for you. You are better off turning down the job than dealing with legal issues. I have never been sued by anyone but I am very careful about what I put on shirts and I get permission from the company if it is questionable. I have had many people ask me to put "Washington Redskins" on a shirt so I tell them "You get me a letter from the NFL saying I can do it and I will" You never hear back from them.

Harley Davidson has one of the most aggressive legal teams going after copyright infringement...I've had a retail store for over 30 years and know what my distributors go through to get any HD products. Just to 'make' an HD product is $10,000 'fee' PER DESIGN last time I checked.
So do it at your own risk...because if you do one, their buddy is going to want one, and his buddy, and his buddy...
