I have been looking for a form to use when a customer comes in to place an order. Is there a generic one out there somewhere that I can get. Right now we use a small pad of paper and get their name address, etc,.. and what design they want and where they want it. I know there has to be something more professional out there, just haven't found it yet.

There are a lot of quoting applications that provide what you need. One to look at for down the road is www.Tquoter.com.
If you just need forms, SMR offers some stock forms you can customize.
