How to Monogram

Hi everyone,
I am getting orders come in for christmas for monograms. What is the proper way to lay them out? Both for Husband/Wife and for a Single person. Anyone whos knows and can help I would really be grateful.

The "standard" monogram for a single person, where the center letter is larger than the side letters, is first name initial, last name initial, middle name initial.

For a couple, it is man's first name initial, last name initial, wife's first name initial.

Forgot to mention that for the letters all the same size, as on a shirt pocket, it would be first, middle, last.
