Donate Life design

Hi all,
Does anyone have the Donate Life embroidery design? I am doing shirts for a golf memorial/scholarship tournament and they would also like the Donate Life design on the sleeves. They have the rights to use the design but I was hoping to not have to recreate the digi file.

BTW, the Donate Life design I am looking for is not a ribbon. It is a square basically divided in half with DONATE on top, LIFE on the bottom and a swirl and a line in the middle.
