Design puckering on childrens cotton school shirts

I have tried cutaway stalizer, sticky stabilizer and tearaway sprayed with 505. I have about 50 of these shirts to do and will be a on going thing. I have had the digitizer redo for me and I still can't get to stitch out without puckers!

Any help would be appreciated!

I always use a fusible mesh stabilizer with a soft tearaway under that for cotton or knit shirts and never have puckers.
