Standard Damage Waiver

Does someone have a standard damage waiver they can send me. I know someone sent it to me before but I can't find it for the life of me. I think it's time to get organized.

All these years of telling clients they would have to sign one if we did this or that scared people away. Now I have one that actually wants it done anyway!

Also, if you don't mind me posting it to the site here (in generic form) that would be awesome as well - though not required if you don't want me sharing it.
