Hoop Question

Someone please tell me how to enter a hoop so I can see if I did something wrong. I bought a square 11.5in hoop from Melco some time ago and entered it a 11.5 X and Y with a 5/8 width and .5 offset. I have used the hoop before and it was fine. Now, it will not center. Off to the right 1/2".
